Continuing Healthcare

We work with local NHS Integrated Care Boards (ICBs) to deliver continuing healthcare services to individual clients in a variety of settings outside of hospital. Such as in their own home or in a care home.

Our teams of dedicated and specialist care professionals provide tailored packages of care up to 24 hours per day, to people with ongoing and significant physical and/or mental health needs, who have been assessed by the NHS.

Continuing Healthcare is not condition specific, but typical tasks can include assisting with:

  • Breathing
  • Personal care
  • Medications
  • Mobility such as repositioning, moving and walking
  • Pressure area care
  • Communicating
  • Eating and Drinking
  • Managing a complex or degenerative condition as part of a multi-disciplinary team
  • Challenging Behaviour
  • Seizures

We also provide fast track continuing healthcare to individuals with rapidly deteriorating health.

We are always looking for

Dedicated and experienced Care Assistants

We are always looking to add dedicated and experienced Care Assistants to our hard working continuing healthcare teams. Experience of continuing healthcare is preferred but not essential and condition-specific training will be provided for free (as applicable to each individual package of care).

Career progression opportunities are available. Many of our Seniors, Care Co-ordinators, Care Supervisors and Managers started as Care Assistants.
